- Students already in the Faith Formation program: parents/guardians & students are asked to read the policy handbook (on the website) and sign the Commitment Form. Parents/guardians and students must sign the form, also be sure to give the grade the student is entering. Signing the commitment form will ensure that your child/children is/are enrolled in Faith Formation for 2024-2025 (one form per family).
- New families, or students please fill out the New Student Registration Form, (one form per student).
- Send payment along with the registration form. If you have more than one child in the program, you may send one check.
- Return the commitment/registration form and payment to the parish office as soon as possible.
